Seam bubbles in drywall.

Our ceiling seams in a bonus room have bubbled out after being fixed five times thus far in two years (new construction). The builder is at a loss as to the cause. What is going on?

Is it the Drywall Tape or Mud Compound that is bubbling out? This is a tough question and only a few ideas that I can think of but it will depend on what is bubbling out.
